When Isaac uses this item and presses one of the fire keys, he will fire a spray of his tears in the direction he fires, in a matter similar to Monstro. The item recharges 5 seconds after it is used.

- Dr. Fetus: Fires four bombs instead of tears.
- Ipecac: Fires five green bomb shots instead of tears.
- Brimstone: Fires one large laser and two smaller lasers at a slight angle. The larger laser destroys rocks.
- Technology: Ditto, but with Technology beams instead of Brimstone lasers.
- Epic Fetus: Acts as Doctor's Remote, but four missiles will drop on the selected area instead of one.
- Mom's Knife: Throws a salvo of five knives.
- Soy Milk (Rebirth): Fires a very large amount of very weak tears.
- Chocolate Milk: The fire key can be held to charge the item up. The more the item is charged, the more tears that will be fired.
- Parasite: Launches one large, short-ranged, orange tear, which spews numerous smaller ones in every direction when it hits the ground, a wall, or an enemy.
- Polyphemus: Shoots a large tear among the smaller ones, which has the same range as Isaac's.
